This visa is issued to foreigners who are professors and teachers of educational institutions, doctors and nurses of hospitals, scientists, professionals, and other workers in banking, commercial, industrial, agricultural and other business enterprises.

1. Notarized Letter Request  of the Applicant-organization
   
2. Notarized and Accomplished General Application Form (BI Form No. MCL-07-01)
   
3. Department of Labor and Employment (DOLE) certified true copy of Alien Employment Permit (AEP)
   
4. Bureau of Immigration (BI) Clearance Certificate
   
5. Clear copy of the applicant’s passport showing the admission and authorized period to stay

Additional Requirements for Accompanying Spouse or Unmarried Minor Children

1. NSO-certified copy of the marriage contract and/or birth certificate of the unmarried minor children
2. a Philippine Embassy or Consulate authenticated copy of the English translated marriage certificate and/or birth certificate, if written in foreign language.
3. Clear passport photocopies
