This visa is issued to a foreigner who:

1. is a member of a religious denomination that is a bona fide non-profit organization in the Philippines
2. is a minister or a religious worker of a non-profit religious denomination
3. is requested by his religious organization to carry out a religious occupation in the Philippines
4. is going to study a fixed curriculum in order to accomplish and undergo a proper formation training

Requirements

1. Notarized Letter Request  from the petitioner-organization with the details on the applicant’s place of assignment
   
2. Notarized and Accomplished General Application Form (BI Form No. MCL-07-01)
   
3. Certified true copy of the SEC Certificate of Registration, Articles of Incorporation and the By-laws of the petitioner-organization
   
4. Certified true copy of missionary credentials
   
5. Bureau of Immigration (BI) Clearance Certificate
   
6. Clear copy of the applicant’s passport showing the admission status and the authorized period to stay

Additional Requirements for Accompanying Spouse or Unmarried Minor Children

1. NSO-certified copy of the marriage contract and/or birth certificate of the unmarried minor children
   
2. a Philippine Embassy or Consulate authenticated copy of the English translated marriage contract and/or birth certificate if written in foreign language
   
3. Clear photocopies of the passports
